CLEP are $40.00 for each exam. ACT-PEP-RCE fees are $75.00 each for HA&P and MB, $90.00 each Nursing exam and $175.00 for the English exam. (Paid only at time of that individual exam)

IMPORTANT: These estimated costs are a current approximation and are subject to change at the discretion of the University of the State of New York (USNY), CLEP, or ACT-PEP-RCE, without notice. Costs in Section I added to the costs in Section II equal the total approximate cost to you.

FINANCING YOUR CPNE AND USNY ENROLLMENT FEE

In our College Advancement Plan your CSMs will be sent to you at a time designed especially to meet your best interests. We will direct you when to enroll in Regents College, USNY and when to apply for your CPNE (clinical), plus where you can elect to take your 2 1/2 day CPNE. Your subject order can be structured so your Enrollment and CPNE Fees are due at the same time, if you so desire. This allows us to be of the best use for your financial needs, if you chose to use our Optional Financing services at that time.

COMPLETE THE FOLLOWING (Select either 5, 10, 15, 20, or 25 for the payment day, with the first payment not over 45 days from the date below): PAYMENTS DUE ON THE th DAY OF EACH MONTH BEGINNING IN , 1998 AND CONTINUING THE SAME THEREAFTER UNTIL THE BALANCE IS PAID IN FULL.

INSTALLMENT NOTE: THIS IS AN AGREEMENT FOR THE PURCHASE OF STUDY MATERIALS AND SERVICES AND AN AGREEMENT THAT CLIENT WILL NOT (PRESENTLY OR IN THE FUTURE) COPY, SELL, "SHARE", OR DISTRIBUTE SUCH STUDY MATERIALS WITHOUT THE PRIOR WRITTEN PERMISSION OF H. G. Kidd Corp. (Hereafter referred to as "Corporation"). This is an agreement by and between Corporation and the above named party (hereafter referred to as "Client"). The parties to this agreement have contracted for the purpose of Corporation supplying the Client with study materials and related services pertaining to helping the Client achieve the college credits for an (AAS(n)) Associate in Applied Science (Nursing) Degree. That Client agrees to properly prepare for, allowing Corporation to guarantee the passing of, the subject exams administered by CLEP and/or ACT-PEP. That Client has provided Corporation the correct information concerning any previous college credits as needed by Corporation to properly "assess" the needs for completion of Client's AAS(n) Degree, with the understanding that these requirements are controlled by Regents College, USNY. That Corporation does not control any "other Costs" beyond the materials and services that Corporation provides. THAT ALL PAYMENTS RECEIVED BY CORPORATION ARE NON-REFUNDABLE (except as noted below). THAT ALL CONDENSED STUDY MODULES (CSM's) RECEIVED BY CLIENT ARE TO BE PAID FOR IN FULL. That Client agrees to pay Corporation in the timely manner set forth. That Client agrees to pay Corporation a $5.00 late payment fee (5 days) and/or $20.00 bad check fee, if such occasions occur. That Client and/or Corporation may cancel this Agreement with a 30 day written notice, without further payments required (within the conditions stated herein).

GUARANTEE: Client has a full money back guarantee of the cost of the CSM (as per average cost of CSM - see above) if client fails to pass the subject exam twice and has provided proof of such failure and the complete subject CSM is returned to Corporation. This does NOT include CPNE materials.

SPECIAL CONDITIONS (IF ANY): -- That Client understands their State Board policy and that one of the enrollment requirements of USNY, Regents College must be met, or obtained, before enrollment in USNY, Regents College. --


Corporation reserves the right to refuse business to anyone. That this Agreement is not valid until approved by the Home Office of H. G. Kidd Corp.
